Columbia Bank was established in 1912 under the name Columbia County Bank. Over the years, it underwent several significant changes, most notably changing its name to Columbia Bank on July 1, 2006. The main office of the bank was relocated multiple times, first moving to 127 West Hillsboro Street in Lake City, Florida, on May 9, 1977, and later to 173 Northwest Hillsboro Street in Lake City on July 1, 2003. After more than a century in operation, Columbia Bank merged with Millennium Bank in Ooltewah, Tennessee, on June 1, 2020, marking the conclusion of its independent operations.
